If you are struggling with a health condition or any aspect of your life that is affecting your health and wellbeing - including practical or lifestyle issues or stress, anxiety or depression - you may benefit from the support of a health coach.
It doesn't matter if you don't know what would help, your health coach will work with you to establish what's most important to you and help you to achieve it.
Anyone over 18 and registered with a local GP practice can have up to five FREE health coaching sessions with a dedicated health coach.
The sessions are between 45 minutes and an hour long at a time and location to suit you.
You will also be given access to the online self-management platform that provides additional tools and support.
